Referring a Patient

All new patient referrals should be directed to our Reno office. To refer a new patient to Nevada Retina Associates, please click here to download the Doctor’s Referral form, fill it out completely, and fax it to our Reno office along with the Dr.’s notes for expedited triage and scheduling.

If the patient’s condition is emergent, we recommend calling in the referral.
